主旨：有關簡化新式戶口名簿版本一案，自103年12月1日起實施，請 查照。
說明：
一、依據內政部103年11月26日台內戶字第10312016792號函辦理。
二、依據內政部103年8月7日研商簡化新式戶口名簿版本相關事宜會議決議略以，為不影響戶役政資訊系統運作效能，同時兼顧民眾使用之便利性，戶口名簿以「現住人口、省略記事」作為核發基準，再由申請人選擇是否列印非現住人口或記事。
三、自103年12月1日起，取消戶口名簿版本名稱(不再列印甲式、乙式、丙式或丁式)，戶口名簿以核發「現住人口、省略記事」為基準，另申請人並得選擇增加同一戶長戶內之非現住人口或詳細記事（態樣如附件）。
四、有關版本簡化前核發之戶口名簿（甲式、乙式、丙式及丁式），仍可供查驗使用。配合簡化新式戶口名簿措施，戶政資訊系統查詢103年12月1日起核發之新式戶口名簿請領紀錄（3100）作業畫面，將載明「含記事」、「現住」、「含非現住+記事」、「含非現住」，以資識別。
