五、請各校承辦處室協助事宜：(一)影印發放講座通知單（含
報名表）(附件)。(二)完成家長報名統計表(附件)並於10
3年10月24日前回傳，傳真：03-3245491，以利本校彙整
統計人數製作簽到簿。
六、首次參加之家長於簽到時發予學習護照，日後參加其他場
次，請攜帶本護照以累記認證章，家長可依教育學堂課程
表選擇參加，同一年度參與4場以上者，將由桃園縣政府
頒發「學習楷模」獎，連續二年度參與4場以上者，發予「
學習典範」獎。
七、教師請於103年10月31日中午前上教師研習系統「山腳國
小」登錄報名，全程參與核予3小時研習時數。
八、為響應環保，請自備環保杯具。
九、參加本場次講座之家長，贈精美紀念品乙份。
