一、旨揭活動相關資訊摘要如下：
(一)參加對象：申請104年度補救教學實施方案學習扶助學校，得推薦參與補救教學優秀教師參加。
(二)補救教學活動設計：請以A4直式橫書、格式不限，須包含以下內容：單元活動名稱、教材來源與分析、設計者、學習目標、符合分段能力指標、教學活動流程簡要、補救教學建議策略、時間分配及評量方式等。
(三)補救教學影片錄製：教學影片光碟請錄製剪接15分鐘精華版1份 (應清楚呈現師生互動、學生操作與教材呈現之畫面內容)。
(四)送件時間：104年9月15日至9月30日止，請繳交補救教學活動設計內容及作品授權書各1份，其電子檔、教學影片及相關資料燒錄成光碟，一併寄送桃園市樂善國民小學教導處蘇盈彰主任收（地址:桃園市龜山區樂善村11鄰樂安街71號）。
二、各校參與補救教學活動設計與影片錄製徵件活動情形，列入年度輔導訪視特色指標項目；另各校所送教學活動設計與教學影片錄製完整者，經評審通過得請領授權費每件新臺幣600元整。
