二、請詳閱「臺中市立臺中第一高級中等學校 111學年度科學班 甄選入學因應嚴重特殊傳染性肺炎(COVID-19)疫情甄選生及家長注意事項」。
三、請自行下載填妥附件1-2「臺中市立臺中第一高級中等學校 111學年度科學班甄選入學因應嚴重特殊傳染性肺炎(COVID -19)疫情切結書」務必隨報名文件一併寄至台中一中。
四、請自行下載附件1-1「因應嚴重特殊傳染性肺炎(COVID-19)健康關懷表(甄選生)」請務必填寫完整，並於科學能力檢定當日(3月12日星期六)繳交給校門口工作人員。
五、應試當日請應試學生務必全程配戴口罩並配合量測體溫，為 落實防疫工作及減少人潮聚集，除試務人員及甄選生外，不開放甄選生親友進入校園內陪試。
六、身心障礙、突發傷病甄選生可申請1位親友陪試；集報之國中學校可申請師長陪試，以協助甄選生所需的服務，前述皆請填陪試申請書暨同意陪試通知書 (如附件1-3)，於報名期間111年2月21日至3月4日掛號郵寄本校申請，經審核同意者由本校以電子信件寄送，陪試人員於甄試當日攜帶「陪試申請書暨同意陪試通知書」、本人身分證件並填寫健康關懷表(如附件1-4)，經工作人員查驗後，始進入校園內陪試，陪試人員請配戴口罩、量測體溫(若有發燒，禁入校園陪試)，儘可能待在開放空間。如屬居家隔離、居家檢疫或自主健康管理(被限制不可外出)者，不得陪試。
七、提醒甄選生，應試前，若被衛生主管機關列管為「自主健康 管理」(無症狀)對象，請於接獲通知後，立即與本校試務中 心聯絡(連絡電話04-22226081轉262)，試務中心將協助安排甄選生至「備用試場」應試。
