| 一、 | 依據教育部國民及學前教育署補助辦理十二年國民基本教育精進國民中小學教學品質要點。 |
| 二、 | 徵件主題：生命教育教學融入式課程教材及教案設計。（各領域教學活動設計時，將生命教育之概念，融入教材設計、活動實施或評量之中，即為融入式教案設計。） |
| 三、 | 徵件參加對象資格：桃園市公私立國民中學現職教師、代理、代課教師及實習教師。 |
| 四、 | 報名及收件期間：自即日起至104年12月24日24時止（以郵戳日期為憑）。 |
| 五、 | 徵件參加名額及件數限制： |
| (一) | 每件參賽作品之作者可採合作方式進行，每件最多2人。 |
| (二) | 每校務必提送參賽作品1件，且以1件為限(由各校推選參加)，每校超過1件或超過上限件數將作為備取件數。 |
| 六、 | 獎勵辦法： |
| (一) | 送件作品經評選榮獲特優者，每人核予嘉獎1次；榮獲優等者，每人核頒獎狀1張；榮獲甲等者，每人核頒獎狀1張。 |
| (二) | 榮獲特優、優等、甲等之教師另列本市「國民中小學教育人員著作給分審查要點第7點第10款」，採計著作分數0.1分，由二人以上合作者，著作分數給分依作者人數平均之。 |
| (三) | 成果發表：參賽作品將收集於成果輯中，並製作成光碟分送各校，以達教學資源共享之目的。 |
