(一)請於活動辦理完畢二週內，將統一收據(繳納人：桃園市立瑞坪國中)、「黏貼憑證」及「匯款帳號明細表」寄瑞坪國中（桃園市楊梅區瑞坪里文化街535號）吳韻芝教師 收(二)請於活動辦理完畢一個月內，將成果冊儲存成Word文件檔，簽到表、研習當天相關講義及教師個人學習成長心得等紙本資料掃瞄成PDF，成果照片6張依序上傳至http://ceag.tyc.edu.tw/ceag_school/school.php →「108學年度國中小教師優良教學示例到校分享成果冊」。
(三)上述資料如附件二，請自行下載使用。
