二、 為增進民眾與四健會員對於四健會的了解及認同，該會發行電子票證形式之四健會認同卡，藉由競賽邀集民眾以藝術創作的方式設計認同卡，表現四健會之精神及價值。
三、 報名方式：請先至線上報名表單（http://bit.ly/designcard1）完成資料填寫及圖檔上傳後，填寫報名表、參賽設計圖、著作使用授權同意書等資料寄該協會(地址:10089台北市中正區辛亥路一段37巷一號)。
四、 收件日期：自即日起至108年11月21日止。
