2.報名資格：
(1)治療中或是已康復之癌症病友，罹癌時間2年以上。
(2)各類癌別皆可，需診斷證明資料。
3.報名方式：
(1)繳交資料：
1. 抗癌心情故事 / 志願服務計畫 / 推薦函（詳見網站格式範本）
2. 病理證明
3. 其他相關檢附文件
4. 生活照六張 (個人照2張、家庭或團體照4張，限jpg檔，像素300dpi或1MB以上，並依
照片之人事時地物簡要說明)
(2)欲報名者，請備齊上述資料後，至「抗癌鬥士徵選活動」官網報名區，上傳檔案資料，網址如右：https://cancerfighter.canceraway.org.tw。
(3)如因故無法上傳檔案，請先來電洽詢本會「抗癌鬥士活動小組」，申請郵寄開放與登記。
4.評選方法：
(1)初審：書面審查，以徵選資料之完整性為主。
(2)複審：以被推薦人之求生正面度、觀念正確度、助人行動力、故事啟發性及推薦資料等作為評分標準。
(3)決審：專人訪查，經由評審團審核選出10名抗癌鬥士。
